Appointments

We look forward to collaborating with you in your care. We welcome both established and new patients to our practice. We participate with most insurance plans (please see below).

Secure VTGyn Patient Portal: Current Vermont Gynecology patients can log in and request an appointment (as well as review their health information and send non-urgent questions and refill requests). Communications through the portal are confidential and secure. VTGyn Patient Portal

Our receptionist will ask you questions so we can schedule the appropriate type of visit. (Please note that, given insurance company constraints, problem visits need to be scheduled as separate visits from annual preventive exam visits. We dislike this as much as you do, but insurance companies tie our hands on this.)

Please arrive 15 minutes before your appointment time to complete any paperwork.

If you are transferring your care to Vermont Gynecology from another practice, please complete a records release form and send it to the other practice, requesting them to send us your records.

If you are new to our practice, we invite you to print out and complete your paperwork prior to your appointment: see Initial Intake Form; Patient Information Form and Request for Medical Services (to be completed after reading Notice of Health Information Privacy Practices). If you are returning, we invite you to complete our Interval Intake Form. You may mail your completed paperwork to us at Vermont Gynecology, 1775 Williston Rd., Suite 110, So. Burlington, VT 05403); fax it to us at (802) 862-9637; or bring it with you to your appointment.

Cancellations

If you are unable to keep your appointment, kindly call the office at least 48 business hours in advance to cancel or reschedule so that others may use the time we had reserved for you. Note that fees will apply to no shows and late cancellations. Please take a moment to review our Late Arrival, and No Show/Late Cancellation policies.

Discharge Policy

We at Vermont Gynecology are honored to provide care for our patients and value our long term patient relationships. For that reason, we are committed to working with our patients to resolve issues that arise in the course of our care.
